      Thanks for letting me know John! I’ll be sure to fix it right away! As to the food diary, I don’t have a form per say. I use the Lose It! and FitBit apps to track my food. If you want to do it old school, buy a small notebook and track every morsel that goes in your mouth. Write down what it was how much and calorie amount at a minimum. You can go a step further and also write down your mood, you might see a pattern emerge as to when and why you snack! Good luck!
